Ellis & Adams was retained by Vanguard Health Systems to manage the design and implementation of a portfolio of 40 IT systems over a 24 month engagement to support a population health and wellness health system New Braunfels, TX.
Resolute Health is a new health and wellness concept providing innovative healthcare programs and services that help patients take control of their own health and wellbeing. The hospital was designed to work under a risk based contracting and population health strategy, architected to enhance the patient experience, improve outcomes and reduce costs to change the health of the hospital’s community. The health system strategy included a proactive and holistic approach to healthcare as the key to minimizing and preventing illness, providing healthy opportunities to encourage total wellness in the Comal, Hays and Guadalupe tri-county community.
The system architecture included implementing an integrated ambulatory practice management system to support operations of 5 outpatient centers to providing primary, endocrinology and urgent care. The design also including integrating a full suite of acute care systems to support a 110 bed facility providing emergency, cardiovascular, orthopedics, women’s services, stroke care, surgical care and oncology services to the community of patients. The hospital also prepared for roll-out of a provider owned health plan with integrated IT services for membership management, actuarial and risk stratification.
Over a two year period, Ellis & Adams was responsible for designed & delivering the following:
Develop a project governance model to include a cast of 80 team members including leadership from the hospital, subject matter expertise across an array of 20 different vendors, clinical and financial members of the operational team who would eventually work as directors and managers of business lines within the hospital
Design and manage a project management office to oversee a $100 million portfolio of IT investments from infrastructure design to patient engagement
Develop a project charter and scope to organize and phase the simultaneous design of all workflows within the hospital. This including door to door simulations for anticipated patient conditions for the health needs of the community
Design and implement a data migration strategy for the acquired outpatient centers
Design and deliver on a comprehensive education program to provide workflow, care delivery and IT education to a staff of 500 personnel across revenue cycle, acute and ambulatory care.
Design & lead the opening support staff of over 150 personnel to field discoveries and resolve issues encountered with the simultaneous activation of all hospital IT systems
The health system began receiving patients in October of 2013 at the endocrinology and urgent care centers. Patient data from acquisitions of primary care facilities were successfully integrated into the system with operations migrated to the new standardized care models. The acute care hospital opened its doors in May of 2014, receiving accreditation from The Joint Commission 60 days after opening.
